Your 2000 DHS probably only has a trans cooler - you can tell - if there are steel lines in BOTH plastic tanks - or just one side
The plastic tanks are replaceable by a radiator shop - NOT something you can do yourself - however - it probably isn't worth putting tanks on a 12 year-old core - just replace the entire radiator.
While you're at it - check both radiator hoses - if either extremely hard or soft - replace them. Also check ALL the heater hoses - pay particular attention to those hoses with 90 degree bends in them - feel the outside of the bends - if they feel soft - replace them.
When you refill the new radiator - use a 50 / 50 mix of DEX and DISTILLED water - pick up a couple gallons at the grocery store - and DO NOT use any type of stop-leak.
